The skin is the largest and most visible organ of the body it weighs 6-8 lbs and has a surface area of around 20 square feet.
The skin is made up of two main layers:
Beneath the dermis is the subcutaneous layer, this provides support to the dermis and stores fat which protects the internal structures.
The skin has 6 main functions:
As we age certain functions of the skin diminish leaving the skin more vulnerable to damage:
Y croen yw organ mwyaf a mwyaf amlwg y corff. Mae'n pwyso 6-8 pwys ac mae ganddo arwynebedd o tua 20 troedfedd sgwâr.
Mae'r croen wedi ei wneud o ddwy brif haen:
O dan yr isgroen mae'r haen isgroenol; hwn sy'n cynnal yr isgroen ac yn storio braster sy'n diogelu'r strwythurau mewnol.
Mae gan y croen 6 phrif swyddogaeth:
Wrth i ni heneiddio mae gweithgarwch y croen yn lleihau gan adael y croen yn fwy agored i niwed:
There are 4 stages to the wound healing process. Most of them will overlap each other, however having a brief understanding will allow better decision making in regards to the choice of dressing and recognising wound response.
Ceir 4 cam i'r broses o wella clwyf. Bydd y rhan fwyaf ohonynt yn gorgyffwrdd â'i gilydd ond bydd rhywfaint o ddealltwriaeth yn arwain at wneud penderfyniadau gwell o ran dewis y gorchudd ac adnabod ymateb y clwyf.

Signs and symptoms of infection include:
Some patients for example diabetic patients may not display obvious signs of infection.
Mae arwyddion a symptomau haint yn cynnwys:
Efallai na fydd rhai cleifion megis cleifion â chlefyd melys yn dangos arwyddion amlwg o haint.
Asepsis – Being free from pathogenic bacteria in sufficient enough numbers to cause infection.
Decontamination –Removing or killing pathogens on an item or surface to make it safe for handling, reuse or disposal by cleaning, disinfection and or sterilisation.
Cross-infection – The transfer of bacteria from one site to another.
Healthcare-associated Infection (HCAI) – Any infection acquired by a person as a consequence of healthcare interventions regardless of where care is delivered.
Sterile – Free from all live bacteria or other micro-organisms.
Micro-organism (microbe) – Any living thing (organism) that is too small to be seen without the aid of a microscope. Bacteria, viruses and some parasites are microorganisms.
Mode of transmission – The way that micro-organisms spread from one person to another. The main modes or routes of transmission are airborne (aerosol) transmission, droplet transmission and contact transmission.
Asepsis – Bod yn rhydd rhag bacteria pathogenaidd mewn niferoedd digon uchel i achosi haint.
Dad-ddifwyno – Cael gwared ar neu ladd pathogenau ar eitem neu arwynebedd i'w wneud yn ddiogel i'w drin, ei ailddefnyddio neu waredu trwy lanhau, diheintio neu sterileiddio.
Croes-heintio – Trosglwyddo bacteria o un safle i un arall.
Haint cysylltiedig â Gofal Iechyd (HCAI) – Unrhyw haint a gaiff person o ganlyniad i ofal ymyriadau gofal iechyd waeth pa le cafwyd y gofal.
Di-haint – Yn rhydd o bob bacteria byw neu ficro-organebau eraill.
Micro-organeb (microb) – Unrhyw beth byw (organeb) sy'n rhy fach i'w weld heb gymorth microsgop. Mae bacteria, firysau a rhai parasitiaid yn ficro-organebau.
Dull trosglwyddo – Y ffordd mae micro-organebau yn lledu o un person i un arall. Y prif ddull neu lwybr trosglwyddo yw trosglwyddiad yn yr awyr, (erosol), trosglwyddiad trwy ddiferion a throsglwyddiad trwy gyswllt.
Any break in the skin increases the risk of bacterial invasion. The risk of wound infection increases according to the type of bacteria, the amount of bacteria present and how the individual responds to the bacteria (host response).
Certain patients and wounds will be more at risk from infection such as larger wounds that have been present for a long duration. The position of the wound may also increase risk.
Patient risk factors are:
Some potential sources of wound contamination are:
Mae unrhyw doriad yn y croen yn cynyddu'r risg o ymlediad bacteria. Mae'r risg o haint yn cynyddu yn ôl y math o facteria, faint o facteria sy'n bresennol a sut mae'r unigolyn yn ymateb i'r bacteria (ymateb y claf).
Bydd rhai cleifion a chlwyfau yn wynebu mwy o risg o haint megis clwyfau mwy o ran maint sydd wedi bod yn bresennol ers cryn amser. Gall safle'r clwyf hefyd gynyddu'r risg.
Ffactorau risg claf yw:
Ffynonellau posibl halogi clwyfau:
It is important to understand your role and responsibility if you suspect a wound has become infected. The signs and symptoms of the infection that you are observing within the wound bed should be clearly documented.
A qualified health care professional such as a Doctor or nurse should reassess the wound and decide on treatment choices.
These treatment choices could be:
*Wound swabs are not always necessary and should only be considered if the wound is not responding to antibiotics or topical antimicrobial dressings.
Look at antimicrobial dressings that are available in your area.
What are the Pros and Cons of these dressings?
How long should I use these dressings for?
Mae'n bwysig deall eich rôl a'ch cyfrifoldeb os ydych yn amau bod clwyf wedi ei heintio. Dylid dogfennu'n glir yr arwyddion a'r symptomau o'r haint rydych yn gweld yng ngwely'r clwyf.
Dylai gweithiwr iechyd proffesiynol cymwys megis Meddyg neu nyrs ail-asesu'r clwyf a phenderfynu ar y triniaethau i'w dewis.
Gallai'r triniaethau o ddewis fod:
*Nid yw swabiau clwyf yn hanfodol bob amser a dylid ond eu hystyried os nad yw'r clwyf yn ymateb i wrthfiotigau neu orchuddion gwrth-ficrobig argroenol.
Edrychwch ar rwymau gwrthficrobaidd sydd ar gael yn eich ardal.
