Prior to re-assessment of the wound, check current treatment plan and previous wound documentation outlining its presentation.

Prior to any wound care, all equipment and treatments should be checked for expiry date and its condition, ensuring that there are no breaks in the seal or any risks of cross contamination.

• Is there an appropriate surface for me to place my dressing pack and sterile field?
• Can the patient gain access?
• Are there any manual handling considerations, such as assisting the individual onto the bed?
• Does any Personal Protective Equipment (PPE) need to be used as per infection control policies?
• Ensuring the room is at a comfortable temperature should the patient need to undress.

Depending on wound site, the patient may need to remove clothing. Maintaining their privacy and dignity is paramount. A lockable door, offering a chaperone and the use of curtains around the bed and window is necessary. It may also be necessary to aid the individual into a comfortable position, ensuring the wound is easily accessible.
